The US Food and Drug Administration (FDA) has approved the injectable dermal filler Radiesse (Merz North America, Inc) for hand augmentation to correct volume loss in the dorsum of the hands, the company announced June 4.

Earlier this year, as reported by Medscape Medical News, the FDA’s General and Plastic Surgery Devices Panel of the Medical Devices Advisory Committee voted 9 to 4 that the benefits of Radiesse outweigh its risks. A majority of panelists also voted that it is safe and effective.

“Radiesse provides an immediate volumizing effect and can help to reduce the prominence of tendons and veins in the hands, delivering smooth, natural-looking results that can last up to 1 year,” the company said.

Radiesse is an opaque dermal filler composed of synthetic calcium hydroxylapatite microspheres suspended in a water-based gel carrier. It was first approved in the United States in 2001, and has since had subsequent approvals, including for correction of nasolabial folds in 2006. It has been used off-label for hand augmentation — and is approved in 52 countries for that indication.

Data to support FDA approval for hand augmentation came from a multicenter, randomized controlled study of 114 patients. Most were white women with an average age of 53 years. Eighty-five patients were randomly assigned to immediate treatment, and 29 to delayed treatment and were considered controls. The control group crossed over to treatment at 12 weeks.

The effectiveness of Radiesse was measured by the Merz Hand Grading Scale (MHGS) — validated by the company in another study — and the Global Aesthetic Improvement Scale (GAIS), which the patients used to self-assess results.

According to the MHGS, 75% of Radiesse patients experienced at least a one-point improvement at 3 months compared with 3% of those in the control group. The mean change in the MHGS from baseline was 1.1 point for those given Radiesse compared with 0.1 in the control group.

In addition, 98% of treated patients reported improvement in the appearance of their hands at 3 months. Improved aesthetic outcomes as measured on the GAIS after initial and repeat treatments correlating with clinical improvement were demonstrated in this study, with all primary and secondary endpoints being met, the company said.

Most adverse events were injection site reactions such as swelling, redness, pain, and bruising, which were usually mild to moderate, short in duration (lasting about 1 week), and required no treatment. No severe device-related adverse events were reported that required treatment, according to the company.

Radiesse is contraindicated for patients with severe allergies manifested by a history of anaphylaxis, or history or presence of multiple severe allergies; patients with known hypersensitivity to any of the components; and patients with bleeding disorders.

June Featured Medspa Service of the Month: Laser Hair Removal

For some, having an excess of body hair or hair in undesirable places, can damage our confidence and make life difficult and public appearances embarrassing.

There are many perfectly natural reasons why body and facial hair grows the way it does, where it does – but luckily, a visit to our spa can help reduce this problem from your life – forever.

Depending on your skin type and health status, laser hair removal treatments can be extremely beneficial. The redcuction of unwanted hair can take up to six sessions, with intervals of up to a few months between each session. How many sessions will I need?

Since hair follicles grow and regrow in a cycle and only growing follicles are affected by treatment, even the most sophisticated laser hair removal treatments require multiple sessions to ensure full effectiveness. The number of treatments vary but can range between six to twelve treatments, with sessions separated by six to twelve weeks depending on the area being treated.

It is completely normal for hair to continue to grow after your laser hair removal treatment. Some of these hairs are being expelled from the newly disabled treated follicles, and some are from dormant follicles entering their active phase after the laser hair removal treatment. This normal cyclical nature of hair growth is why our Laser Hair Removal Protocol consists of a series of laser hair removal treatments per area, scheduled six to twelve weeks apart. This is why any laser hair removal therapy that purports to be effective using fewer treatments is making impossible claims.

How long is each session?

Each session is customized to the individual, their skin type, hair color, etc. The size of the area will also affect the length of the session. For instance, a lady’s underarms and a gentleman’s back have a significantly different surface area. Sessions can generally last between 15 and 45 minutes.

Is laser hair removal painful?

At Natura Medspa we use an FDA approved technology available which combines radio frequency (RF) with Diode laser energy. The use of RF allows us to optimize our energy requirements for minimum discomfort. In addition we use Synercool technology which effectively cools the skin surface during treatment thus providing a soothing experience.

How does it feel like?

Most patients report no more discomfort other than a quick pinching sensation. Each pulse, which lasts less than a second, produces a slight tingling feeling. In particularly sensitive areas, such as the upper lip, may feel like a quick, light pinch.

Is there anything I need to do to prepare for a session?

We both share a responsibility in the success of your treatment. As with any procedure, there are some steps you can take both before and after your treatments to ensure maximum effectiveness while mitigating any potential for complications.

Avoid tanning or especially sun-burning the areas to be treated prior to treatment. While tanning and sunburns are always tough on your skin, they can decrease the effectiveness of the treatment or even cause unnecessary discomfort.

For the most effective treatment, ensure that the area to be treated is shaved before your treatment. Waxing, tweezing, and bleaching the hair can lessen the effectiveness of your treatment.

Also, avoid putting any kind of lotion, sunscreen, makeup, perfume or any other type of product on the skin being treated to avoid potential complications. They can block the laser energy or even, in some rare cases, cause allergic reactions.

Are there any post treatment considerations?

We both share a responsibility in the success of your treatment. After your laser hair removal treatment, it’s normal for the treated area to be pink or slightly red for a day or so after treatment. A soothing lotion can help address those minor discomforts and keep the skin adequately hydrated. If you practice good skin care, drink plenty of water and avoid excessive exposure to direct sunlight and your skin should return to normal very quickly. On very rare occasions, pinkness, redness, minor blistering or persistent pain may occur. If they do, contact Natura Medspa for instructions.

Who are the best candidates for hair removal?

Anyone with unwanted hair in any part of the body is a potential candidate for laser hair removal.
Many men and women choose to remove hair from multiple areas in the body. Facial hair, underarm,
bikini lines, womens legs and men chest and backs are often treated.

We customize the treatment to exactly match your hair and skin color and type.
As long as there is some color in your hair (even blond hair has enough color), then our system will be effective.

Are there any side effects?

The energies used by Natura Medspa are both commonly used in a range of medical and cosmetic procedures that have proven to be safe. Most people experience no side effects at all, though few exhibit some short-term local reddening of the surrounding skin. You can return to your regular activities immediately after treatment, although it is advisable to keep the skin protected from exposure to direct sunlight for a short period of time.
